What is ERP therapy for children with OCD?

Exposure and Response Prevention (ERP) is a form of cognitive behavioural therapy (CBT) specifically adapted for OCD. It helps children and young people gradually face situations, thoughts or triggers that cause anxiety while learning not to respond with compulsions, avoidance or reassurance-seeking. ERP is a recommended psychological treatment for children and young people with OCD.

Yes. For children and younger adolescents, parents or carers are often actively involved in treatment. They can learn how to support exposures, reduce unhelpful reassurance or accommodation, and reinforce the skills their child is developing. NICE recommends that CBT including ERP for children and young people involves family members or carers and is adapted to the child’s developmental age.

Yes. OCD can involve intrusive thoughts, images or impulses as well as compulsive behaviours. ERP can be adapted to address mental compulsions, avoidance, reassurance-seeking and other responses that maintain the OCD cycle, rather than focusing only on visible rituals.

Online therapy can provide a flexible way of delivering CBT and ERP, including when attending a clinic is difficult. However, treatment should still be individually assessed and adapted to the child’s developmental stage, OCD symptoms and family circumstances. NICE recommends that CBT including ERP is adapted to the developmental age of the child or young person.

There is no fixed number of sessions that applies to every child. The duration depends on the severity and complexity of the OCD, the child’s age and developmental needs, treatment goals, and how they respond to therapy. A clinician will normally review progress throughout treatment and adjust the plan accordingly.
